Patient Satisfaction Surveys

We may ask you to share your opinion about the care that you received by completing a survey either during or after your discharge from service. This survey is generally mailed to your home,
Only client names, addresses and visit dates are used to send surveys.
Your feedback will be used to help us evaluate the care and services you received, and to improve how we provide client care in the future.
